Completing a Salary / Rota Adjustment form

The Salary Adjustments form can be found on the right-hand menu of Evolve.

The following describes how you would complete each section on the form

Section 1 – Employee Details, Method of Payment.

Employee Detail

  • Select an Employee
    • Find the relevant employee, list is sorted alphabetically by forename. The dropdown box will include all staff from your service
  • Service and Authorised By are automatically filled in.
  • Payment Method
    • Adjust Next Pay – Employee will be paid next payday
    • Bank Transfer – Employee will be paid by Bank Transfer as soon as possible
    • Rota Update Only – No payment/recovery required

Section 2 – Missing Shift/Rota Activities

  • Click on Add Line

  • Choose the correct date, start time, and end time.
  • Choose the Activity Type
    • All rota activities are available
  • Choose Person/Team Being Supported
    • This is a free text box, please ensure that correct information is entered
  • No of Hours
    • This field is automatically calculated based on information you input.
  • Deduct from Salary
    • Use this check box if hours recorded are to be removed from employee's next pay.
  • Bin Icon
    • Use this to remove line if information is incorrect.

Section 3 – Others (ANNUAL LEAVE, ABSENCE ETC.)

  • Click on Add Line

  • Choose Type
    • All absence and leave activity types are available
  • Choose correct start date and end date.
    • If the absence or leave is for 1 day, the start date and end date must be the same.
  • No of Hours – Choose the correct number of hours using the arrows to increase and decrease.
  • Deduct from Salary
    • Use this check box if hours recorded are to be removed from employee's next pay.
  • Bin Icon
    • Use this to remove line if information is incorrect.

Section 4 – Apply

 

Apply button

  • Once all relevant and correct information has been added, click on apply. After you click on 'Apply' you should see the following pop up on screen. This confirms that the Salary Adjustment has been successfully completed and sent to the relevant people.

Application successful

If you don't see this message, check at the top of the form for any errors. Update any missing information and hit apply again.
